The assessment form collects the pool type, your name, a mobile number, and the pool address. It may also record which page you submitted from and the time of the request.

PoolVana uses that information to follow up and, if the pool is in the service area, to schedule a Pool Health Assessment. It is not sold.

Above-ground pool selections are not sent as leads. A hidden field is used to ignore automated submissions.

If a request cannot be delivered, the form says so. It does not pretend the request was received.
